Achievement and Improvement Recognized by MNBTE

 Congratulations to six IHN clinics for their improvement and/or achievement in 2011 QI initiatives.  Minnesota Bridges to Excellence, MNBTE recently announced they were recognizing the following clinics for excellence in providing care to patients with diabetes, vascular disease or depression.

IHN clinics being recognized are Duluth Family Practice Center for Improvement in Diabetes Care, Cromwell Medical Clinic for Achievement in Diabetes Care, Raiter Clinic for Improvement in Depression Care, Northland Family Physicians for Improvement in Vascular Care, Longville Lakes Clinic for Improvement in Diabetes Care and for Improvement in Vascular Care, Lake Superior Internal Medicine for Improvement in Vascular.

To receive an achievement reward a clinic must be among the top performers in the state.  To receive an improvement reward a clinic must demonstrate marked improvement one year to the next.

These rewards represent hard work and diligence on the part of the providers and staff of these clinics and set an example of excellence.

Clinics Get Help Introducing New Health-Care Model

Seven clinics that are members of IHN are getting a little help in adapting a new health-care model favored by the state.  Buffalo Clinic, Cromwell Medical Clinic, Iron Range Clinic, Northland Family Physicians, Raiter Clinic, Range Medical Clinic and St. Cloud Medical Group all received $1,000 to help them earn Health Care Home status.

Health Care Homes are being developed as part of health reform legislation passed in Minnesota in 2008.  Patients with chronic illnesses or disabilities are cared for in innovative ways, including contacting patients via phone and/or email between appointments.  Costs associated with achieving the status include purchasing software to help track patient progress.

Neither the $1,000 grant nor the fees clinics are allowed to charge comes close to covering a clinic's costs, said Rachael Nyenhuis, chief operating officer for IHN. "But it's the right thing to do for patient care. Patients with complex health conditions often can get lost between the cracks.  This is a way to make sure someone doesn't fall off the radar between visits. 

Studies show health-care homes - known in some states as medical homes - also result in long-term savings because patients' problems can be addressed before they require more expensive care, Nyenhuis said.

So far, 134 clinics have been certified as health-care homes, according to the Minnesota Department of Health's website.

Intergrity Health Network Scores Another Win...

Intergrity Health Network scores another win for reducing patient costs.

Integrity Health Network has received another award from the HealthPartners “Partners in Excellence” Program for achieving lower costs for services versus the state average, while maintaining quality benchmarks.

To be eligible for this award, IHN scored below what is known as the average Total Cost Index for Minnesota. This index is calculated by measuring the network’s use of resources and the total cost of providing care across the network.

The quality ratings are based on HealthPartners data collection and data reported by Minnesota Community Measurement, a nonprofit that creates measures for, and reports on, improvement initiatives in the state’s health care industry.

“This marks the second year running that Integrity Health Network has been awarded for cost-containment efforts,” said Bruce Penner, IHN’s Director of Quality. “Clinics across the network contributed to the final results, proving once again reducing patient costs is a constant, network-wide commitment.”

Integrity Health Network is the largest, most comprehensive network of independent care providers in the region. Independent clinics are not owned by a corporate system, but by the physicians who live and work in their communities.

HealthPartners is the largest consumer-governed, nonprofit health care organization in the nation providing care, coverage, research and education to improve the health of members, patients and the community. It’s headquartered in Bloomington, Minn.

Duluth Kidney Services and Fairview University Partner for Transplant Patients

University Kidney Transplant Program, in coordination with Duluth Kidney Services, is doing an outreach post-transplant clinic every other month, at the offices of Duluth Kidney Services. Transplant Surgeon Dr. Ibrahaim Hassan and one of the transplant coordinators see patients for their yearly post-transplant followup, as well as if interim visits with the transplant program are needed. Plans are in the process to eventually do pre-transplant workups here, so that patients do not need to travel to Minneapolis until the time of their transplant.

IHN Honors Outstanding Performing Clinics

Integrity Health Network recognized three clinics for outstanding performance at our recent annual meeting.

The Integrity Health Network 2011 Achievement Award was given to the primary care clinic that demonstrated the highest level of achievement within the network for a publically reported quality improvement measure. The award went to Northland Family Physicians for their outstanding accomplishment of attaining a 48% rate for Optimal Diabetes Care.

The Integrity Health Network 2011 Improvement Award was given to the primary care clinic that has demonstrated the greatest percentage of improvement over one year within the network for a publically reported quality improvement measure. This award went to Iron Range Clinic for their outstanding improvement in Optimal Diabetes Care, improving from 12% to 34% in one year.

Finally, our 2011 Specialty Clinic Award was given to a specialty clinic that demonstrated its commitment to the value and importance of quality improvement programming in specialty care through its self-initiation and consistent investment in quality. This award went to Northland Ear, Nose and Throat Associates for their innovative and comprehensive process for monitoring and improving the care and follow up of patients with head and neck cancer and patients receiving chemotherapy.

Congratulations to the physicians and staff of these three clinics for setting an example of Integrity’s mission and vision.

UCare Honors Integrity Health Network with Quality Performance Award

IHN Receives UCare Quality Award!

Integrity Health Network was recently recognized by UCare as a top-performing healthcare organization in Minnesota. A recent press release by UCare states, “This network of independent-minded primary care and specialty clinics in northern Minnesota and northwest Wisconsin offering a plethora of caring solutions was recognized by UCare’s P4P program for Medicare Overall Performance Quality of Care.”

Congratulations and a big “Thank You” to all of the physicians and staff across our network that give of themselves every day in providing the excellent care that this award recognizes.  

St. Cloud Medical Group Providers Achieve Health Care Home Certification
